WHO WE WORK WITH

Working alongside the people around each person.

Established in 2013 by front-line workers, Harmony supports youth, families, and adults at home in Fredericton and surrounding areas, working within the plans set by the professionals involved.

Social workers

Most youth and family referrals come from social workers with the Department of Social Development. The social worker develops the care plan. Harmony provides support in line with it.

Other professionals

Schools, health and community organizations are welcome to contact us to talk through whether Harmony may be suitable and how a referral would be made.

Families and individuals

Call us about home support or with general questions. For youth and family services, the professional referral process applies, and we can explain how it works.

FROM ENQUIRY TO AGREED SUPPORT

How a referral works.

An enquiry opens a conversation. Acceptance, availability, and the support arrangement are confirmed with Harmony management.

Make an enquiry

Call us or send a referral enquiry online with general information only: your role, the support being considered, the general location, and timing.

Talk it through

A member of Harmony management calls you back, usually within two business days, to discuss the request, service fit, and current availability.

Share details securely

If Harmony is a good fit, we agree a secure way for the social worker to share the care plan and the information support staff need.

Match and plan

We match a support worker with suitable experience and availability, and confirm the schedule, start date, and communication arrangements.

Support begins

Introductions are made and support starts, delivered in line with the care plan and the responsibilities agreed with the social worker.

Stay in touch and review

The social worker receives updates through the agreed channel, concerns are raised promptly, and the arrangement is reviewed as needs change.

Keep detailed information secure

Please do not put names of youth, case files, care plans, diagnoses, or sensitive family information in a website enquiry or voicemail.

Once Harmony is confirmed as a good fit, we agree a secure channel with the assigned social worker before any case details are shared.

  • Raises concerns promptly with the social worker

The young person and family

  • Are listened to and treated with respect
  • Know who their support worker is and when to expect them
  • Can share feedback with Harmony management at any time

WHO WILL PROVIDE THE SUPPORT

Screened, trained, and supported.

Before working with youth and families, Harmony staff must provide a clear Criminal Record Check, including a Vulnerable Sector Check, and a Social Development Prior Contact Check. They also need current Standard First Aid and CPR.

New staff complete a paid orientation and ongoing training, and Harmony management is available to them when a situation needs guidance, so the people you refer get consistent, well-supported care.

Before you refer.

Who can make a referral?

Most youth and family referrals come from social workers with the Department of Social Development. Other professionals are welcome to contact us to talk through whether Harmony may be suitable and how a referral would be made. Families and individuals can call about home support or with general questions.

What area do you serve?

Fredericton and surrounding communities. If the person you are referring lives outside the city, ask us about coverage for that location when you first get in touch.

How quickly can support start?

It depends on current capacity and on finding the right match. We will give you a realistic timeframe during the first conversation. If the need is time-sensitive, tell us when you first contact us.

Can support happen on evenings or weekends?

Scheduling, including evenings and weekends, is discussed during the referral conversation and depends on staff availability for the support requested.

What should I include in the online enquiry?

General information only: your name, role and organization, how to reach you, the type of support, the general location, and timing. Please do not include names, case details, or health information. Those are shared later through an agreed secure channel.

Is Harmony an emergency service?

No. If someone is in immediate danger, call 911. For urgent child protection concerns, contact your local Department of Social Development office or After-Hours Emergency Social Services.
